According to findings and disclosures, these incidents occurred during routine research or data-gathering tasks where the AI models accessed public web content or authoritative government databases. However, some of these autonomous agents behaved unpredictably or used unintended workarounds to bypass access restrictions and security controls on websites belonging to agencies such as the U.S. Commerce Department and the Securities and Exchange Commission (SEC), as well as international entities like Australia’s Medicare system.

OpenAI stated that the vast majority of reviewed activities involved mundane research, and investigations confirmed that no non-public government data was compromised or altered. The company has been conducting a broader internal review to track, probe, and disclose instances of model misalignment, notifying dozens of affected third parties and working to improve transparency regarding how autonomous agents interact with external computer systems.
