First officer warned that Amazon cargo plane was coming in too fast before deadly Miami crash, NTSB says, tell me about this news

A Boeing 767 cargo aircraft operated by 21 Air for Amazon Prime Air overran the runway during a landing at Miami International Airport, resulting in a fatal crash that killed five people on the ground.

The National Transportation Safety Board (NTSB) released preliminary findings focusing on cockpit communication and the aircraft’s approach profile:

  • Repeated Warnings from the First Officer: Cockpit voice and flight data recorders revealed that the first officer repeatedly warned the captain that the plane was coming in “too fast” starting more than a minute before touchdown. The first officer also called for a go-around to abort the landing, but the captain did not verbally acknowledge all of the speed cautions.

  • Excessive Speed and Deep Touchdown: The aircraft crossed the runway threshold at over 200 mph (well above standard approach speeds) and floated past nearly 40% of the runway, finally touching down roughly 3,750 feet past the start of the pavement at a groundspeed around 160 to 185 mph.

  • Incomplete Braking and Deceleration: Flight data indicated that neither the speed brakes (spoilers) nor the thrust reversers were deployed to decelerate the aircraft upon landing. Manual wheel braking was initiated, briefly released when an attempt to abort (go-around) was initiated with less than 1,000 feet of runway remaining, and then reapplied too late to prevent the overrun.

  • Ground Impact and Casualties: The aircraft rushed off the end of the paved runway at high speed, plowing through airport perimeter fencing and striking two vehiclesincluding a work van where five occupants diedbefore catching fire. Both pilots survived with injuries.

The NTSB’s investigation is examining crew resource management (CRM), airline operating procedures, wind conditions, and why the landing was not aborted earlier.
